35-year-old Ivan McGuire filmed the jump from 10,000 feet, but after leaving the plane, he realized that he had forgotten to put on his parachute…
The unfortunate incident occurred in April 1988 when McGuire was making his third jump of the day and was excited to film a skydiving lesson. He grabbed a camera to film the student and instructor, and within seconds he jumped with them, not realizing his fatal mistake.
According to the Daily Star, McGuire had made a total of 800 successful jumps before the fatal accident. The terrifying footage shows him approaching the ground and preparing to open his parachute until he realizes with horror his huge mistake.

McGuire’s last words were: “Oh God, no!” His body was later found in the woods, a mile and a half from the airport from which he took off. At the time, pilot Mark Luhmann was under investigation to determine whether he checked that McGuire had a parachute. The inspector involved in the case said: “There is a rule that no one can jump unless the pilot checks the parachutes.”
